We designed for the full complexity of living with a chronic condition, connecting every touchpoint across the care journey rather than solving for isolated moments. Biologic therapies treat chronic inflammatory conditions effectively yet healthcare fragmentation causes 40-60% of patients to abandon treatment. Through research with 1000+ patients, caregivers, and clinicians across 5 countries, we designed a care ecosystem integrating nurse champions, AI-powered progress visualization, injection confidence rituals, physician dashboards, and automated renewal coordination. Patient app launched in 2024; Service Design Vision under review.
